Medical Form for Sick Passengers
If you have certain medical needs, you must complete the MEDIF form with your treating doctor and submit it at least 48 hours before your flight. This helps ensure your safety and comfort during travel.
Who needs to fill out the MEDIF form?
You must complete the form if you:
- Require a stretcher, oxygen, a medical escort, or treatment during the flight
- Have recently had an illness, treatment, or surgery that may affect your fitness to travel
- Need to carry or use medical equipment onboard
When should the form be submitted?
At least 48 hours before your scheduled flight. The form should be emailed to the Air Arabia Medical Center (AAMC) at medreport@airarabia.com.
Who should sign the form?
Both the passenger and their treating doctor must complete and sign the MEDIF form.
How long is the Fit to Fly certificate valid?
The Fit to Fly certificate, issued by the Air Arabia Medical Center (AAMC), is valid for 7 days from the date of issue, unless a shorter validity is specified in the authorization letter.
What if the certificate expires before travel?
If the certificate expires, a new one must be obtained before you can fly.

You can click here to download the MEDIF form. Once completed and signed by your doctor, please email it to medreport@airarabia.com at least 48 hours before your flight.
What if my condition changes before the flight?
If your health changes, you must inform Air Arabia immediately. You may need to submit an updated MEDIF form or a new Fit to Fly certificate.
Can a family member submit the form?
Yes. A family member or authorized representative can submit the completed form on your behalf.
Additional requirements
Passengers requiring wheelchair assistance or oxygen support must indicate this clearly on the form and make arrangements in advance with Air Arabia. These requests will also be reviewed as part of the Fit to Fly authorization process.
